Any data we collect is obtained directly from patients during patient visits or, if requested, through text messages. Personal information is kept in the patient medical record, which is accessible only to Steven Wager, MD. No one else has access to these records. Demographic data (e.g., address, date of birth) and medications prescribed are also recorded in our electronic prescribing system, which complies with all HIPPA regulations.
Use Of Personal Information: We use personal information only to provide medical care. We also send text messages to remind patients of upcoming visits or outstanding balances. Personal information is not used for any other purposes.
Disclosure Of Personal Information: Information is disclosed only with the consent of the patient and in the following specific situations. 1) if information is requested by your insurance company, we disclose information only if we receive
written consent from the patient. 2) We will disclose information if we communicate with other clinicians involved in patient care (e.g., physicians or therapists). However, we only communicate with other clinicians if requested by the patient and with the expressed consent of the patient. 3) There may be times when it is helpful to communicate with family members. We only communicate with family members after we receive the expressed consent of the patient, and we only disclose information the patient has given us permission to disclose. We never communicate with family members unless the patient gives us explicit consent to do so. We do not disclose information to anyone else under any other circumstances. SMS (text messages) opt-in and phone numbers for the purpose of SMS will not be shared with third parties and affiliates, or anyone else, for marketing or any other purposes.
